WTB: Blank Maintenance Record for USA and Canada

Does anyone have or know if this in photo maintenance record is available to buy could really use one for USA and Canada only as my car never came with one and the PO's didn't have it. Got taken with a few other parts that were not nailed down when I bought it 35 years ago.

    Thanks for any leads

Good replica booklets without the bourgundy cover are offered on the UK ebay site regularly. They are made by a DDK porsche forum.
