What Grill Type & Year is this from?

Hello,

My car is a very early 1969 LWB T, could someone help and advise what type of Grill this is and what Year,
I don't think its a 69?

You're correct- the five bar grill started in 1970 and went through 1971. Late 1968 through 1969 used three bar grill with corresponding decklid.
